Example: Purchase 5 qualified headsets and return 5 used headsets and receive 5 x $15.00 = $75.00.

from The Headset Team during the promotion period.
You must complete the online claim form. Upon the customer making a qualified purchase The Headset Team will email customers (within a business day in most cases) regarding where customers need to go to complete the online claim form to receive your Rebate.
When customers place an order a proof of purchase order confirmation will be automatically sent to the email address provided when the customer placed their qualified product order. You will need this when you complete the online claim form.
For each rebate requested, return an equal number of used headsets by requesting a pre-paid shipping label required for shipping the used headsets via the online form.
Limit up to three hundred (300) rebates per customer.
Offer valid for purchases of Qualified Products from The Headset Team in accordance with these Terms and Conditions. Rebate requests for purchases from other retailers or channels will not be fulfilled. This offer cannot be combined with any other offer or discount from The Headset Team or Plantronics. Qualified Product(s) must be purchased during the Promotion Period and rebate requests must be submitted within 28 days of invoice date.
Rebates will be issued to you via debit card or check within 28 days of validation of your claim. All rebates will be paid in USD.
The “Trade-in Offer” is open only to legal residents of the 50 United States, District of Columbia who are 18 years or older. If all the terms and conditions of this Rebate Offer are not met, the rebate will not be issued.
Please keep copies of all materials submitted, as you may be required to resubmit upon inquiry.
Inquiries related to non-payment of rebates must be made no later than April 30th, 2016.The Headset Team & Plantronics accepts no responsibility for lost, late, damaged or misdirected rebate requests or shipments. If a customer cannot be contacted for any reason, such as incomplete or inaccurate information, the customer will not receive the rebate. Rebates will not be mailed to PO boxes. This offer is not valid with any other The Headset Team or Plantronics sponsored offers. Void where prohibited or restricted by law. The Headset Team and Plantronics reserve the right to verify and audit rebate requests and may disqualify such requests if purchase of the Qualified Products cannot be substantiated to The Headset Team or Plantronics satisfaction. To substantiate purchase of the Qualified Products The Headset Team and Plantronics may require you to provide the UPC code from the product packaging. Fraudulent submissions will not be fulfilled and may result in prosecution. Submissions become the property of The Headset Team and will not be returned.

Additional program terms and conditions may apply that are present on the online claim form. The Headset Team reserves the right to cancel or modify the program or reject a claim at any time for any reason.
